Met Council is proud to serve as a trusted community partner in New York's Social Care Networks, a groundbreaking initiative that represents a new way of caring for Medicaid members, one that addresses the root causes of poor health before they become medical hardships.
By connecting eligible New Yorkers with nutrition services, housing-related services, screening, eligibility assessments, and enhanced care management, we help our neighbors avoid unnecessary hospitalizations, manage chronic conditions, and live longer, healthier lives.
Through our participation in Social Care Networks across the five boroughs, including SIPPS, SOMOS, and WholeYouNYC, our work is centered on what matters most: measurable health outcomes for the communities we serve.
We serve New York Medicaid members in neighborhoods across all five boroughs, from Flatbush to Far Rockaway, Mott Haven to Washington Heights to the North Shore of Staten Island.
The Policy

A historic effort to transform how Medicaid serves New Yorkers.

The Social Care Network program operates under New York State's Medicaid Section 1115 Waiver, a historic demonstration approved by the federal Centers for Medicare & Medicaid Services (CMS), an agency of the U.S. Department of Health and Human Services led by Secretary Robert F. Kennedy Jr.
The waiver reflects a simple but powerful insight: when health-related social needs like food insecurity and housing instability go unmet, they lead to avoidable emergency room visits, hospital stays, and preventable suffering, at great cost to both individuals and the healthcare system as a whole.
By meeting those needs early and in the community, this program is designed to improve health while reducing unnecessary utilization and lowering costs across the system. This is the first coordinated statewide effort of its kind, and it aligns with the vision of Governor Kathy Hochul and Commissioner Dr. James McDonald of the New York State Department of Health to transform how Medicaid cares for the New Yorkers it serves.

Our Role

Your partner from screening through ongoing care.

Under New York's 1115 Medicaid Waiver, Social Care Networks (SCNs) are responsible for connecting Medicaid members to health-related social services. SCNs partner with Community Based Organizations (CBOs) like Met Council to do the hands-on work of serving members directly.
As a CBO, Met Council handles the full member journey. We screen members to identify their social needs. We assess their eligibility for specific programs under the Waiver. We navigate them through their options, explaining what they qualify for and enrolling them in the programs they choose.
When members qualify for services we do not provide directly, we refer them to the right organization. And we deliver services directly in two areas: nutritious food and home safety.
Beyond service delivery, we provide ongoing care management. We do not just sign you up and walk away. As your needs change, we reassess, connect you to additional programs, and continue supporting you through the services you are enrolled in.
We report outcomes back to the Social Care Network so each member's care stays coordinated across providers. Our goal is simple: remove barriers, deliver services, and make sure no one falls through the cracks.

The largest public health nonprofit in New York City and a lead Social Care Network, coordinating screening and service delivery across three boroughs. Met Council partners with PHS to deliver screening, nutritious food, pantry referrals, home safety modifications, and care management.
