Medicaid-Covered Care That Meets You Where You Live

As a Community Based Organization partnering with New York's Social Care Networks, Met Council does more than deliver services. We screen you, assess your eligibility, navigate you into the right programs, refer you to other organizations when needed, and continue supporting you over time.

We also provide direct services in two areas: nutritious food and home safety.

Where It All Starts

We help you figure out what you qualify for and get you there.

Before any services are delivered, Met Council works with you to understand your needs, determine what programs you are eligible for, and guide you through enrollment. This is the foundation of everything we do.

When you qualify for services Met Council does not provide directly, we refer you to the right organization and make sure the connection sticks.

Needs Screening

We sit down with you (by phone or in person) and screen for health-related social needs, including food insecurity, home safety risks, respiratory health concerns, and other challenges. This is a conversational process designed to understand your full situation, not just check boxes.

Eligibility Assessment

Based on your screening, we review the programs available under the 1115 Waiver and identify which ones you qualify for. This includes services Met Council provides directly (nutritious food and home safety) as well as services offered by other community organizations.

Program Navigation and Referral

We explain your options, answer your questions, and handle enrollment for the programs you choose. If you qualify for services outside of what Met Council offers, we refer you to the right provider and follow up to make sure you are connected.

Ongoing Support

Your needs can change over time. As we work with you, we continue to reassess and connect you to additional programs and resources as they become relevant.

Nutritional Food Services

Nutritious food that fits your life, your health, and your culture.

Met Council operates one of the largest kosher and halal emergency food networks in the country. Through the Social Care Network program, we deliver culturally appropriate nutritious food to eligible Medicaid members at no cost. Here is what we provide:

Food-insecure members are 47% more likely to be hospitalized than those who are food-secure. Nutritious food is not just a social service. It is a proven path to better health outcomes and lower healthcare costs.

Prepared Meals

Medically tailored, religiously certified meals designed for your dietary and cultural needs, whether kosher, halal, or otherwise. Meals are delivered directly to your door on a regular schedule.

Grocery Boxes

Curated boxes of nutritious groceries including produce, protein, and shelf-stable staples, tailored to your health and cultural requirements. Grocery boxes are also delivered to your home.

Pantry Access

Direct referral to the nearest of our 200+ partner pantries across all five boroughs. Scheduling and intake are handled for you, and every pantry provides a welcoming, culturally competent experience.

Benefits Navigation

Food is often just the starting point. Our team helps eligible members identify and enroll in other programs available to them, connecting you to Met Council's wider network of benefits and social services.

Home Safety Services

Making your home safer, healthier, and easier to live in.

For Medicaid members at risk of falls, injuries, or respiratory illness in their homes, Met Council provides non-invasive home safety services through the Social Care Network program. These services are designed to help you live independently and safely, without requiring major construction or renovation.

Every year, 1 in 4 Americans aged 65 and older falls. In 2024, over 43,000 older adults died from preventable falls, and the healthcare system spent $80 billion treating fall-related injuries. Simple modifications save lives.

In-Home Safety Assessment

A trained assessor visits your home to evaluate fall risks, accessibility barriers, and indoor environmental health concerns. The assessment covers both mobility safety and respiratory triggers, and it is the starting point for all home safety services. It is cost-free and designed to be quick and comfortable.

Mobility Safety

For members at risk of falls or mobility-related injuries, we provide non-invasive modifications to key areas of the home:

Handrail Installation

Handrails installed in bathrooms, hallways, stairways, and other high-risk areas to provide support and reduce the risk of falls. All installations are non-invasive and do not require structural changes.

Non-Skid Surface Application

Non-skid surfaces applied to floors, bathtubs, and other slippery areas to prevent falls. This is one of the most effective and affordable safety modifications available, especially for seniors.

Asthma Remediation

For members with asthma or respiratory conditions triggered by indoor environmental factors, we provide remediation services to reduce exposure to common triggers:

Carpet Steam Cleaning

Deep cleaning of carpets to remove dust mites, allergens, pet dander, and other embedded particles that contribute to asthma symptoms and respiratory distress.

HVAC Air Filter Replacement

Replacement of heating and cooling system air filters to improve indoor air quality and reduce circulating allergens, dust, and irritants.

HEPA Vacuuming

Thorough vacuuming with HEPA-filtered equipment to capture fine particles that standard vacuums miss. Particularly effective for members sensitive to dust, mold spores, and other airborne triggers.

Ongoing Care

We do not just sign you up and walk away.

After your initial screening and service enrollment, Met Council continues to be your partner. Care management is our commitment to staying with you as your needs evolve, making sure your services are working, and connecting you to additional support when it is needed.

Ongoing Service Support

If you are receiving nutritious food or home safety services through Met Council, we stay in touch to make sure everything is working. If a delivery is missed, a modification needs adjustment, or something is not right, we are here to fix it.

Periodic Reassessment

Your circumstances change. A new health condition, a move, a change in household size. We periodically reassess your needs and update your service plan so you are always connected to the right programs.

Additional Service Enrollment

As we work with you, we often identify new programs you qualify for that were not part of your original enrollment. We handle the navigation and enrollment for those additional services, whether they are provided by Met Council or by another organization.

Coordinated Reporting

We report outcomes and service updates back to the Social Care Network so your care stays coordinated across every provider involved. Nothing falls through the cracks because every part of your care plan is tracked and communicated.
